AIM: To assess irrigant penetration in curved canals after shaping procedures ex vivo. METHODOLOGY: Thirty extracted mandibular molars with moderate to severe root canal curvature were included. A special aiming device was used to guarantee that each successive radiograph was taken with the same positioning. The mesiolingual canal of each tooth was instrumented with ProTaper rotary files. Apical third preparation was completed with an F1 instrument before additional step-back enlargement using F2 then F3 instruments. For each apical taper, the teeth were submitted successively to active irrigation (AI) then to passive irrigation (PI). AI consisted of a 0.5 mL flush of sodium diatrizoate (Hypaque 50%) immediately followed by manual mechanical activation with a gutta-percha point. PI consisted in flushing the canals with sodium hypochlorite passively delivered with a syringe. A digital radiograph was taken after each modality. The influence of needle tip design, needle tip insertion level, irrigant volume, root canal taper and solution activation was assessed by using digital subtraction radiography and measures of the depths of irrigant penetration. Comparisons were performed within an analysis of variance framework in a repeated-measures approach. RESULTS: For PI, all the four explanatory variables ‘apical taper’, ‘volume of irrigant used’, ‘corono-apical level of needle tip placement’, ‘needle tip design’ had a significant (P < 0.005) influence on outcome of irrigation penetration. CONCLUSIONS: Only active irrigation allowed complete penetration and exchange of irrigating solution. For syringe irrigation alone, the level of needle tip placement in the canal was the most dominating factor. Passive irrigation consisted of a 0.5-mL flush of sodium hypochlorite solution delivered with a syringe through a 27-gauge notched tip needle. A digital radiograph was taken after each modality and stored on computer for subsequent digital subtraction and measures of the depths of irrigant penetration. Comparisons were performed within an analysis-of-variance framework in a repeated-measures approach. RESULTS: The penetration of irrigants was significantly greater for each successive step of the shaping procedure when the two modalities were analysed separately (P < 0.001). The difference between the two modalities was statistically significant for each step of the shaping procedure (P < 0.0001). CONCLUSIONS: Shaping root canals improved both penetration and exchange of irrigant inside the root canal system. Complete renewal of the solution was impossible to achieve with a conventional syringe delivery system and a limited volume of solution. OBJECTIVE: To assess the palatability of iodinated oral contrast media commonly used in abdominopelvic CT and CT colonography (CTC). METHODS: 80 volunteers assessed the palatability of a 20-ml sample of a standard 30 mg ml(-1) dilution of Omnipaque® (iohexol; GE Healthcare, Cork, Ireland), Telebrix® (meglumine ioxithalamate; Guerbet, Aulnay-sous-Bois, France), Gastromiro® (iopamidol; Bracco, High Wycombe, UK) and Gastrografin® (sodium diatrizoate and meglumine diatrizoate; Bayer, Newbury, UK) in a computer-generated random order. RESULTS: Gastrografin is rated significantly less palatable than the remaining media (p<0.005). Omnipaque and Telebrix are significantly more palatable than Gastromiro. No difference existed between Omnipaque and Telebrix. 39% of participants would refuse to consume the quantities of Gastrografin required for a CTC examination compared with Telebrix (7%) and Omnipaque (9%) (p<0.05). CONCLUSION: Omnipaque and Telebrix are significantly more palatable than both Gastromiro and Gastrografin, with participants more willing to ingest them in larger quantities as well as being less expensive. ADVANCES IN KNOWLEDGE: Omnipaque and Telebrix are significantly more palatable iodinated oral contrast media than both Gastromiro and Gastrografin, which has potential implications in compliance with both abdominopelvic CT and CTC. PURPOSE: The aim of this study was to compare the quality of low-volume hybrid computed tomography colonography bowel preparation, using both laxatives and oral contrast, with standard polyethylene glycol lavage. METHODS: The study group consisted of 300 consecutive adults (mean age, 58.3 years) who underwent colonoscopy immediately after positive computed tomography colonography. Hybrid bowel preparation for study group was <1 L in total volume, consisting of osmotic cathartic (sodium phosphate or magnesium citrate) in conjunction with oral contrast (2% barium and diatrizoate). A control group of 300 adults (mean age, 58.3 years) underwent primary colonoscopy after standard 4-liter polyethylene glycol lavage without oral contrast. The prospective preparation quality rating by the endoscopist served as the reference standard. A rating of poor/marginal was considered inadequate and adequate/good/excellent was considered diagnostic. RESULTS: The frequency of inadequate bowel preparation was 4.3% (13/300) in the study group vs 12.3% (37/300) for the control group (P < .001). Specifically, preparation was poor or marginal in 10 and 3 cases in the hybrid cohort, respectively, and in 29 and 8 cases in the polyethylene glycol cohort, respectively. Preparation quality was scored as excellent in 32% (96/300) in the hybrid cohort and 23.3% (70/300) in the polyethylene glycol cohort (P < .05). CONCLUSIONS: At colonoscopy, low-volume laxative-oral contrast hybrid preparations are effective for bowel cleansing, perhaps even more so than polyethylene glycol lavage. Beyond improvements in quality, the low-volume preparation may improve patient compliance and would allow for immediate computed tomography colonography if colonoscopy is incomplete, without the need for additional oral contrast tagging. The aim of the study was to assess the effect of selected isolation methods on the viability and metabolism of bovine leukocytes. The cells were isolated using a Ficoll 1077, Histopaque 1083 gradient and osmotic shock method, and Ficoll or Histopaque with osmotic shock. Evaluation were made of the total number of cells, viability after isolation and in 24h culture on RPMI 1640 medium and metabolism with NBT reduction assay. Microscopic and cytometric evaluation of the leukocytes revealed that the isolation methods applied had an influence on their number and viability. Based on the results it can be concluded that isolation methods of cells in a Histopaque or Ficoll yield highly pure cell fractions with high viability.

OBJECTIVE: The purpose of this article is to determine whether early postoperative CT provides prognostic information in patients with advanced ovarian, tubal, or primary peritoneal carcinoma with optimal debulking reported at primary cytoreduction. MATERIALS AND METHODS: Our study included 63 patients who underwent primary cytoreductive surgery for presumed advanced ovarian cancer, who had optimal debulking (residual disease â‰?1 cm) reported at surgery, and who underwent CT before and 7-49 days after surgery. Two radiologists independently retrospectively interpreted all postoperative CT scans and scored lesions on a 5-point scale, where 1 indicates normal and 5 indicates definitely malignant. Lesions larger than 1 cm with a CT score of 4 or 5 were considered suboptimally debulked residual disease. RESULTS: Suboptimally debulked residual disease on CT (range, 1.1-5.8 cm) was reported by reader 1 for 29 of 63 patients (46%) and by reader 2 for 31 of 63 patients (49%), with substantial interobserver agreement (κ = 0.75). Patients with suboptimally debulked residual disease on CT had significantly worse median progression-free survival (p = 0.001, both readers) and overall survival (p â‰?0.010, both readers). By univariate and multivariate analyses, suboptimally debulked residual disease on CT remained a significant independent predictor of progression-free survival (p = 0.001, both readers) and overall survival (p â‰?0.006, both readers). CONCLUSION: Our study showed that residual disease larger than 1 cm was present on early postoperative CT in almost half of the patients deemed to have optimally debulked disease at primary cytoreduction. Residual disease larger than 1 cm detected on early postoperative CT was associated with significant decreases in both progression-free and overall survival.

OBJECTIVE: The purpose of the study was to determine whether a difference in patient preference exists between iohexol (Omnipaque) and diatrizoate sodium (Gastroview) as oral contrast medium for abdominal-pelvic CT. A secondary objective was to evaluate whether there are significant differences in bowel opacification and adverse effect profile for the two agents. SUBJECTS AND METHODS: From August 2007 through March 2009, 300 patients were enrolled in this prospective study after informed consent was obtained. Eligible patients were identified from those scheduled for outpatient abdominal-pelvic CT. Subjects were randomly assigned to receive one of two oral contrast agents in a double-blinded fashion. Subjects graded the taste using a 5-point scale, and data regarding demographics, total volume, and adverse effects were collected. A direct comparison of 30 mL of each of the two diluted agents in randomized order was then performed. CT images were graded for bowel opacification by two blinded abdominal radiologists. RESULTS: Of 287 subjects who expressed a preference, 233 patients (81%) preferred dilute iohexol compared with 54 patients (19%) who preferred dilute diatrizoate sodium (p < 0.001). Ten patients had no preference, and three patients did not complete the taste comparison study. No difference in bowel opacification was identified between the oral contrast agents (p = 0.27), nor was there a significant difference in adverse effects (p = 0.352). CONCLUSION: Patents preferred dilute iohexol over dilute diatrizoate sodium for oral contrast for abdominal-pelvic CT. There was no significant difference in bowel opacification or adverse effect profile. AJR. INTRODUCTION: Currently, on-line hemodiafiltration (HDF-OL) is the most effective technique. Several randomized studies and meta-analyses have shown a reduced mortality and a direct association with convective volume has been reported. At present, it has not been established if an increased dialysate flow (Qd) results in improved results in terms of convective and depurative efficiency. We aim at assessing the effects of Qd variations on convective volume and its depurative capacity in patients on HDF-OL. MATERIAL AND METHODS: A total of 59 patients (45 men and 14 women) from a HDF-OL programme in which a monitor 5008 Cordiax with self-replacement was used, were enrolled. Patients were assessed in 5 sessions with post-dilutional HDF-OL, using helixone-based dialyzers, with only Qd being changed (300, 400, 500, 600 and 700ml/min). Serum levels of urea (60Da), creatinine (113 Da), β2-microglobulin (11,800Da), myoglobin (17,200Da) and α1-microglobulin (33,000Da) were measured at the beginning and at the end of each session, in order to estimate the percent reduction of such solutes. RESULTS: An increased dialysate volume per session was observed, from 117.9±6.4 L with Qd 300ml/min to 232.4±12 L with Qd 700ml/min. No changes were found in replacement volume or convective volume. Regarding diffusion, Qd increase was associated to a significantly increased dialysis dose, with an increased Kt from 68±6.9 L with Qd 300ml/min to 75.5±7.3 L with Qd 700ml/min (p<0,001), and a gradually increased percent reduction in urea associated to increased Qd with significantly lower levels being found with Qd 300ml/min. No changes were found in other measured substances. CONCLUSION: Qd variations in HDF-OL do not change convective volume. A higher Qd was associated to a slightly increased urea clearance with no change being observed for medium and large molecules. Qd optimisation to the minimal level assuring an adequate dialysis dose and allowing water and dialysate use to be rationalised should be recommended. OBJECTIVE: The purpose of this study is to objectively and subjectively compare nonionic iohexol and ionic diatrizoate iodinated oral contrast agents as part of a cathartic bowel regimen within the same CT colonography (CTC) cohort, with otherwise identical preparations. MATERIALS AND METHODS: In this retrospective study, 46 adults with no symptoms (mean age, 59.4 years; 26 men and 20 women) returning for follow-up CTC over a 9-month interval underwent the same bowel preparation with the exception of 75 mL of iohexol 350 in place of 60 mL of diatrizoate. All other preparation components (bisacodyl, magnesium citrate, and 2% barium) remained constant. Objective volumetric analysis of residual colonic fluid volume and fluid attenuation was performed. Additionally, two radiologists experienced with CTC who were blinded to the specific bowel preparation scored each of six colonic segments for adherent residual solid stool using a previously validated 4-point scale (0 for no stool; 1-3 for increasing residual stool). A paired t test was used for comparison of the cohorts. RESULTS: No clear clinically meaningful difference was found between the two preparations on overall objective or subjective evaluation. The mean (± SD) residual fluid volume was 173 ± 126 mL with the iohexol preparation and 130 ± 79 mL with the diatrizoate preparation (p = 0.02). The mean total colonic stool score was 2.5 (0.42/segment) with iohexol and 2.3 (0.38/segment) with diatrizoate (p = 0.69). The mean fluid attenuation was higher with iohexol (849 ± 270 HU) compared with diatrizoate (732 ± 168 HU) (p = 0.03). CONCLUSION: On the basis of this direct intrapatient comparison, we found that oral iohexol is a suitable alternative to diatrizoate for fluid tagging as part of a cathartic bowel preparation at CTC. Because this nonionic tagging agent is more palatable, less expensive, and likely safer than ionic diatrizoate, our CTC program now uses iohexol as the standard recommended regimen.

RATIONALE AND OBJECTIVES: The purpose of this study was to assess the effect of a low-tube-voltage technique and iterative reconstruction (IR) on the radiation dose and image quality of computed tomography colonography (CTC). MATERIALS AND METHODS: We studied 30 patients (14 women and 16 men; mean age, 64.5 ± 13.1 years; range, 39-90 years) with colorectal cancer referred for surgical treatment. All underwent CTC with fecal tagging under a standard 120-kVp protocol in the supine position and a 100-kVp protocol in the prone position. The 120-kVp images were reconstructed with filtered back projection (FBP). The 100-kVp images were postprocessed using FBP and a hybrid type of IR (adaptive iterative dose reduction 3D). The effective radiation dose (ED), image noise, and contrast-to-noise ratio (CNR) were compared among the three protocols. The visual image quality was scored on a four-point scale. RESULTS: The mean ED was significantly lower under the 100-kVp protocol than the 120-kVp protocol, resulting in a 27% radiation dose decrease (3.5 ± 2.0 vs 2.5 ± 1.5 mSv; P < .01). Image noise decreased by 48%, and the mean attenuation of tagged fluid increased from 452 to 558 HU on images acquired at 100 kVp with IR compared to that in the 120-kVp protocol; these differences were significant. The mean CNR was significantly higher under the 100 kVp with IR than the other two protocols. We found no significant differences in the visual scores for diagnostic utility between the 100 kVp with IR and the 120 kVp with FBP protocol (P = .10). CONCLUSIONS: Low-tube-voltage CTC reduced the radiation dose by approximately 27% while maintaining the image quality.
