Fighting metastasis is a major challenge in cancer therapy, and stimulation of the immune system is of particular importance in the treatment of metastatic cancers. Here, an integrated theranostic nanoplatform was developed for the efficient treatment of highly metastatic tumors. Versatile functions including “”And”” logically controlled drug release, prolonged circulation time, tumor targeting, and anti-metastasis were integrated into doxorubicin (DOX) loaded, highly integrated mesoporous silica nanoparticles (DOX@HIMSNs) for a systemic treatment of highly metastatic triple neg. breast cancer (TNBC). It was found that the good therapeutic effect of DOX@HIMSN was only partially attributed to its anticancer cytotoxicity. Lymphangiomas are congenital malformations of the lymphatic system that consist of cysts of varying size. Although they are benign, they can undergo progressive growth with compression and infiltration of adjacent structures. Surgical excision has been the cornerstone of treatment, although total excision of the lymphangioma can be a major challenge and may be associated with severe complications. Therefore, a variety of nonsurgical methods have been proposed to reduce the surgical morbidity and to decrease the recurrence rate. Percutaneous sclerotherapy of lymphangioma involves the injection of sclerosing substances into the lymphangioma cysts. BACKGROUND: The volume of the gastric reservoir appears crucial to explain the success of laparoscopic sleeve gastrectomy (LSG) in the treatment of morbid obesity. The aims of this study were to describe a new, easy model to estimate gastric reservoir volume after LSG; to evaluate the volumetric changes 1 year after surgery; and to analyze their relationship with weight loss. METHODS: This is a prospective observational study of all patients undergoing LSG in the Department of Surgery at our institution. The gastric reservoir was evaluated radiologically considering the image as a complex geometrical shape with two components: a cylinder (gastric body) and a truncated cone (antrum). Radiologic assessment using this new model was performed at 1 and 12 months after surgery. Moreover, body mass index and percentage of excess weight loss (%EWL) were evaluated at 3, 6, 12, and 18 months after LSG. RESULTS: Forty-five patients (34 F/11 M) with a mean age of 46.9 years were included. A significant increase in total gastric reservoir volume (124.8 ±â€?8.7 and 188.6 ±â€?6.4 mL at 1 and 12 months, respectively; pâ€?â€?.001) was observed. No statistically significant differences were observed comparing volume of the two components at the two time points. The %EWL at 18 months was inversely correlated with reservoir volume changes at 12 months after LSG (pâ€?â€?.006). CONCLUSIONS: We describe an easy volumetric model to estimate the size of the gastric reservoir after LSG. Moreover, a direct relationship between an increase in gastric reservoir volume and a lower weight loss after surgery was documented.

PURPOSE: Endoscopy is recommended to screen for esophageal varices in patients with cirrhosis. The objective of this study was to identify features on abdominal CT imaging associated variceal hemorrhage (VH). METHODS: A case-control study was performed among patients with cirrhosis who had a CT scan. Consecutive patients who experienced VH were included as cases, and patients without VH served as controls. Two radiologists recorded the maximal esophageal varix diameter in addition to other measures of portal hypertension at CT. RESULTS: The most powerful CT parameter associated with VH was the esophageal varix diameter (5.8 vs. 2.7 mm, p < 0.001; adjusted OR 1.84 per mm, p = 0.009). 63% of individuals with VH had a maximal varix diameter â‰? mm compared to 7.5% of cirrhotic patients without VH (p < 0.001). In contrast, the proportion of individuals whose largest varix was <3 mm was 7.4% among VH cases compared to 54.7% among controls (p = 0.001). The varix diameter powerfully discriminated those with and without VH (C-statistic 0.84). CONCLUSIONS: A large esophageal varix diameter is strongly associated with subsequent VH. A threshold of <3 and â‰? mm appears to identify patients with cirrhosis at low and high risk for hemorrhage. Nonoperative management of blunt splenic injury is the treatment of choice in hemodynamically stable patients. Detection of vascular injury by multidetector CT (MDCT) is the most significant factor predicting the need for endovascular treatment. This study evaluated the timing of the appearance of vascular lesions during angiography. Images from 20 patients embolized for pseudoaneurysms (PSA) were evaluated. Angiograms were reviewed for phase and timing of PSA. Admission MDCT was reviewed for injury grade and PSA. Initial MDCT evaluation indicated grade III and IV splenic injuries in 9 and 11 patients, respectively. PSA was seen on MDCT in 14/20 (70%) patients. Time from opacification of the aorta to vascular injury was 1.32 s for arterial phase injuries compared with 2.05 s for postcapillary injuries (P=0.097). Angiography demonstrated 15 vascular injuries during the arterial and 5 in the venous phase. Of injuries seen during arterial phase angiography, 10/15 (66%) were identified on MDCT. Of the five injuries that exhibited postcapillary-phase findings, 4/5 (80%) demonstrated PSA (P=0.5). Vascular lesions are a better indicator of subsequent clinical deterioration than splenic injury grade. PSAs are more frequently seen in postcapillary vascular injuries than arterial phase lesions with the current timing of MDCT. In a subset of patients in whom splenic injury grades III and IV warrant angiography, PSAs are not initially demonstrated on MDCT. Therefore, alteration of MDCT timing parameters to better correlate with arterial phase angiography may improve initial diagnosis of vascular injury.

PURPOSE: The aim of this study was to assess the accuracy of translucency rendering (TR) in computed tomographic (CT) colonography without cathartic preparation using primary 3D reading. MATERIALS AND METHODS: From 350 patients with 482 endoscopically verified polyps, 50 pathologically proven polyps and 50 pseudopolyps were retrospectively examined. For faecal tagging, all patients ingested 140 ml of orally administered iodinated contrast agent (diatrizoate meglumine and diatrizoate sodium) at meals 48 h prior to CT colonography examination and two h prior to scanning. CT colonography was performed using a 64-section CT scanner. Colonoscopy with segmental unblinding was performed within 2 weeks after CT. Three independent radiologists retrospectively evaluated TRCT clonographic images using a dedicated software package (V3D-Colon System). To enable size-dependent statistical analysis, lesions were stratified into the following size categories: small (< or =5 mm), intermediate (6-9 mm), and large (> or =10 mm). RESULTS: Overall average TR sensitivity for polyp characterisation was 96.6%, and overall average specificity for pseudopolyp characterisation was 91.3%. Overall average diagnostic accuracy (area under the curve) of TR for characterising colonic lesions was 0.97. CONCLUSIONS: TR is an accurate tool that facilitates interpretation of images obtained with a primary 3D analysis, thus enabling easy differentiation of polyps from pseudopolyps.

OBJECTIVE: Iodinated contrast media (CM) can potentially cause contrast-induced nephropathy (CIN). It is not clear, however, whether particular types of CM are more prone to cause CIN than others. In this study we compare 4 types of CM (ionic vs. nonionic; monomer vs. dimer) on their effects on the microvessels that supply the area at risk for renal damage in CIN (outer medullary descending vasa recta-DVR). MATERIAL AND METHODS: Using microdissection techniques, single DVR were isolated from rats and perfused using a set of concentric pipettes. After stabilization, perfusate was exchanged for a buffered solution containing either vehicle, or amidotrizoate (an ionic/monomeric CM), ioxaglate (an ionic/dimeric CM), iopromide (a nonionic/monomeric CM), and iodixanol (a nonionic/dimeric CM). The final iodine concentration was 23 mg iodine/mL, a concentration similar to that expected for coronary interventions. At this dilution, properties of CM solutions like viscosity and osmolarity are similar to the vehicle solution. To rule out further influence of CM-osmolarity and viscosity, the DVR bath solution was kept isoosmolar to the perfusate. Angiotensin II dose response curves were performed after the 20 minutes of perfusion. Digital videomicroscopy was used for measurements of luminal diameter. RESULTS: All types of CM reduced luminal diameter of perfused DVR in a similar manner. After 20 minutes of perfusion, size of DVR were: 45% +/- 7% of initial diameter for the amidotrizoate-group; 53% +/- 6% for the ioxaglate-group; 63% +/- 11% for the iopromide-group; and 49% +/- 8% for the iodixanol-group. Control group remained at 96% +/- 4% of initial diameter. The angiotensin II dose response curves showed greater reactivity for amidotrizoate, iopromide and iodixanol, when compared with controls. CONCLUSION: Under conditions where effects of osmolarity and viscosity are kept insignificant, perfusion of DVR using different types of iodinated CM leads to similar constriction of DVR. The response to angiotensin II was enhanced in 3 of the tested CM. This may be an important mechanism in the pathophysiology of CIN.

Accurate measurement of perfusion with dynamic contrast enhanced CT requires an arterial input curve (AIC) uncontaminated by venous sources. Arterio-venous anastomoses (AVAs) are sources of contamination if contrast is injected intravenously. We seek to identify AVAs in mice and associated errors in perfusion measurements. Six transgenic mice with spontaneous prostate tumor were scanned with a micro-CT scanner (GE Healthcare (GE)) using a high resolution anatomical and a lower resolution perfusion protocol. For the anatomical protocol, a CT scan was performed during injection of an iodinated contrast agent (Hypaque) into a tail vein. Images covering the thoracic, abdominal and pelvic regions at an isotropic resolution of 175 microm were reconstructed and rendered in 3D to show the arterial and venous tree (Advantage Window, GE). For the perfusion protocol, each mouse was continuously scanned for 40 s and the contrast agent (Hypaque) was injected via a tail vein 5 s into scanning. Tumor images were reconstructed every second. Tumor blood flow (BF) and volume (BV) maps were calculated with CT perfusion software (GE) using AIC measured either from abdominal aorta (AA) or tail (caudal) artery (TA). In all mice, there was an AVA from the bifurcation of the inferior vena cava to the tail artery shunting venous blood and portion of the contrast agent injected into the tail vein into the TA. Contrast arrival time at the TA preceded that at the AA by 3.3 +/- 0.5 s (P < 0.05). Mean tumor BV and BF values calculated with AA versus TA were 10.0 +/- 1.8 versus 4.8 +/- 2.1 ml (100 g)(-1) (P < 0.05) and 108.8 +/- 26.5 versus 33.0 +/- 8.5 ml min(-1) 100 g(-1) (P < 0.05), respectively. AVA in the murine pelvic region can result in inaccurate and more variable measurements of pelvic organ/tissue perfusion when the tail artery is used as the AIC. PURPOSE: This scoping review assesses the literature and summarizes the current evidence on sclerotherapy for the treatment of lymphatic malformations in pediatric patients. METHODS: A comprehensive search of published and unpublished literature was conducted using multiple databases. Title, abstract, and full-text screening was conducted by 2 independent clinicians. All discrepancies were resolved during consensus meetings. RESULTS: A total of 182 articles were retrieved. Forty-four articles were removed as duplicates, and 11 articles were added after reviewing prominent studies. After full-text abstraction, 44 articles and 2 conference proceedings (N = 882 patients) were included in the final results. Twelve articles were classified as level II and 34 articles as level IV evidence. Picibanil (OK-432) was the primary agent used in most included studies. Postinjection symptoms with OK-432 were primarily fever, swelling, and erythema at the site. Life-threatening complications were uncommon and involved postinjection swelling of cervical lesions causing airway compromise. CONCLUSIONS: The literature regarding sclerotherapy for lymphatic malformations is of a low level of evidence and suffers from a lack of standardization. Randomized clinical trials focused on OK-432, bleomycin, or alcoholic solution of zein; standardized dosing protocols; and consistent and reliable outcome reporting will be necessary for further development of treatment guidelines.
